Baby at week 16
Per twin, week 16 development: Facial muscles allow expressions. Backbone strengthens. Umbilical cord thickens and gets stronger.
Mother's body — what changes
Twin pregnancy at week 16 differs — expect: Quickening (first movements) may be felt, especially for second-time mothers. Round ligament pain intensifies.
Nutrition & supplements
Add ~340 kcal/day above pre-pregnancy baseline. Prioritise iron (27 mg/day), calcium (1000 mg), omega-3 DHA (200–300 mg) and 71 g protein. Continue prenatal vitamin. For twin gestation add an extra 300 kcal on top (total ~600 second trimester, ~750 third), 50% more iron, and split hydration to 3–3.5 L/day.
Safe exercise
Continue 150 min/week. Avoid supine (flat-on-back) exercises after week 20 — they compress the vena cava. Swimming and prenatal Pilates are ideal now. For twins reduce intensity 20% earlier — bed rest is not routinely indicated but modified activity from week 24 is prudent.
